1. Please bring a complete list of all your medications, including the names and dosage, for better understanding of your treatments. Please feel free to bring the actual medication bottles with you.

2. Please stop taking antihistamine for at least three days prior to visit, in preparation of skin allergy testing:

Examples of antihistamine meds include: Claritin (loratadine), Zyrtec (cetirizine), Xyzal (levocetirizine), Allegra (fexofenadine), Pataday eye drop (olopatadine), Alaway eye drop (ketotifen), Astepro nasal spray (azelastine), etc. Montelukast may be stopped for one day prior to visit. DO NOT stop asthma inhalers and other medications such as blood pressure meds.

3. Please bring copy of your insurance card and secondary insurance card you may have.

4. We are open and transparent of the cost of you visit. Copay, co-insurance and/or deductibles are expected to be paid at the time of visit.
